Tomago Aluminium’s Environment, Health & Safety Management System encourages the immediate reporting of all incidents, accidents and injuries across site. This early reporting enables commencement of the investigation process in order to determine root cause and the implementation of preventative actions at the earliest possible stage.

The reporting mechanism is an online system.

 All TAC supervisors are trained in the use of this system.

The normal process for our contractor partners to report is;

1. Contact the TAC Rep managing the work to report the event.
2. In conjunction with the TAC Rep complete the online report and Participate in the investigation as required.
3. Implement any actions required resulting from the investigation.

We recognise that in some circumstances this process may not be able to be followed e.g. when incidents occur getting to and from the job area and there is no direct TAC Rep responsible.

In these instances the following process should apply.

1. Download and print the front page of the investigation form from the  link
2. Fill out the form with all relevant information

4. You will then be contacted by a representative from safety services who will arrange for completion of the form and investigation of the event.
